Failure cells (sidebar: Failure cells, shortcut g f) visualizes the full 2 × 3 taxonomy — completeness (complete / incomplete / extra) × faithfulness (grounded / ungrounded) — and how it moves over time.

Grouping
Toggle how the cells are grouped:
- By cell — all six, individually.
- By completeness — collapse into
complete/incomplete/extrato see the completeness axis on its own.
Reading the split tells you where to spend effort: mostly grounded failures point at retrieval (the context is missing things); mostly ungrounded failures point at generation/grounding (the model is fabricating). That's the same distinction Veralith uses to route heal cards.
